Kiva House Group Home

  • On average 90- to 120-day program, intensively staffed with comprehensive clinical programming including daily skills groups.
  • Licensed as a Behavioral Health Residential Facility (BHRF/previously called therapeutic group home) for eight adults, male or female.
  • Often used as a step down from hospitals, the program works toward the goal of transitioning members to semi-independent, community-based settings.
  • Individual goals are developed based on the member’s treatment plan.

Kiva Semi-Independent Apartments

  • Nine-bed facility consisting of one- and two-bedroom apartments for SMI adults (can have co-occurring diagnoses).
  • Members must have income to pay rent and other costs.
  • Staffed 24/7 to provide support and assistance to members and promote progress and success.
  • Staff are intensively trained and have a strong foundation in psycho-social interventions.
  • Services are delivered in the member’s own apartment to target individual treatment needs and skill development in needed areas.
  • Medication management and assistance with integrated and behavioral health appointments is provided on-site by staff.
  • Most members are engaged in community programs including: school, work programs and off-site behavioral health groups.

Desert Garden: Brief Intervention Program (BHRF)

Desert Garden is designed to provide time-limited, intensive crisis intervention and stabilization services to members with mental health disorders, as well as members with co‐occurring disorders. Treatment, rehabilitation, and psychosocial programming ranges from short‐term care focused on immediate crisis resolution and stabilization, to a maximum of 30 days targeted towards crisis stabilization, diagnosis‐specific education, symptom management and return to outpatient care. Desert Garden BHRF is a 16 bed licensed, co-ed facility that provides a 24 hour therapeutic setting to adults, 18 and older, engaging in a milieu of peer and professional support, specifically designed to divert members from restrictive higher levels of care and coordinate a smooth transition back into the community.

Desert Mountain: Behavioral Health Inpatient Facility (BHIF)

Desert Mountain provides a strong emphasis on psychosocial rehabilitation and recovery services that instill personal responsibility to achieve optimal levels of functioning during their stay and upon discharge. Desert Mountain provides, or ensures access to medical/physical health evaluations and treatment within the scope of the program. The program conducts multidisciplinary assessments conducted by licensed psychiatric and medical practitioners, licensed nursing staff, intake specialists, and behavioral health clinical staff such as therapists or counselors. Nursing services available 24 hours a day, 7 days a week. Desert Mountain is licensed by the Arizona Department of Health Services/Bureau of Medical Facilities Licensure requirements under R9-10 Article 3 BH Inpatient Facilities Rules as a Provider Type (B5) Behavioral Health Inpatient Facility (non-IMD) and provides a 24-hour therapeutic setting to Title XIX eligible adults 18 years of age and older who have a GMH designation or a Serious Mental Illness.

Vida Nueva Independent Apartments

Vida Nueva has 12 one- and two-bedroom apartments, totaling 18 available beds, and is our most independent setting in the adult continuum and fosters the acquisition of life skills which promote self-sufficiency. Independence is gained by residents paying rent and other associated costs (typically allocated from their Social Security Income (SSI), maintaining their apartments, preparing their own meals (with or without staff assistance), studying for classes, engaging in recreational and leisure activities and participating in community-based events.

Residents use public transportation to commute to day programs, schools and jobs, as they need to meet low-income housing eligibility requirements to obtain residence.

Staff personnel are on-site to supervise residents and provide outpatient services ranging from modeling life skills to providing crisis intervention as residents develop independence and achieve individualized goals to transition to independent living.

Skills training is provided daily and is composed of topics like health and wellness, coping skills, healthy relationships and vocational and symptom management/psychoeducation

Referral Requirements:

A comprehensive service provider can make referrals via an Adult Recovery Team if the individual is enrolled in the Arizona behavioral health system.

Chino Valley / Prescott

Operating from offices in Chino Valley, Intermountain Centers has we provide residential support services to adults in Yavapai County diagnosed with developmental disabilities in the form of small group or family residences..Each member’s strengths and needs are assessed and their person-centered team sets their individual goals.By gaining positive life skills, members increase participation in daily self-care and family responsibilities while building communication and social skillsUniversal goals for all members is to foster self-management skills and self-esteem.

Learning opportunities might include acquisition of communication skills for non-verbal members and expansion of vocabulary for verbal members. This comes in the form of: mastering the alphabet, counting numbers, typing and navigating a computer, money counting and transferring, maneuvering through art media, making music and interacting with new people in the community.

Field trips to community-based events, appointments and visits to local restaurants for lunch augment the routine designed to collectively and individually benefit the members.
